As a result, modern chilies evolved defense mechanisms to mitigate the risk of granivorous mammals. While capsaicin is present at some level in every part of the pepper, the chemical has its highest concentration in the tissue near the seeds within chilies. === Eosinophilic asthma === Reslizumab was first used for eosinophilic asthma in 2008. In a 106-patient, phase II clinical trial, the researchers showed reslizumab was effective in reducing sputum eosinophils. Furthermore, the patients receiving reslizumab showed improvements in airway function, and a general trend toward greater asthma control than those receiving placebo was observed. A large, 981-patient, phase III clinical trial showed that reslizumab was effective at improving lung function, asthma control, and quality of life in comparison to placebo. ==== Sniper killings ==== In February 2024, an American doctor returning from the Gaza Strip wrote an op-ed in the Los Angeles Times stating that she had witnessed children being deliberately targeted by Israeli snipers, writing, "On one occasion, a handful of children, all about ages 5 to 8, were carried to the emergency room by their parents. All had single sniper shots to the head... None of these children survived." A Canadian doctor returning from a week in Gaza stated she had seen crimes against humanity, including small children "dying of hunger, bombs, sniper shots." Israeli drone footage released in March 2024 showed Israeli snipers killing an unarmed boy in Jabalia. In April 2024, doctors said they were seeing "a steady stream of children, elderly people and others who were clearly not combatants with single bullet wounds to the head or chest." Dr. Mark Perlmutter, an American doctor working in Gaza, stated Israeli snipers were targeting children, stating, "No toddler gets shot twice by mistake by the 'world's best sniper.' And they're dead-center shots." In August 2024, an American doctor returning from Gaza stated, "We had kids shot in the chest and shot in the head – in other words, clearly deliberate, clearly targeted". === Foot surgeries === Generally, foot surgery is usually reserved for patients with a walking or ambulatory potential. Foot surgery may also be indicated to assist brace and orthosis fitting and hence promote supported standing. The most common foot deformity in arthrogryposis is club feet or talipes equinovarus. In the early years of life the serial casting according to the Ponseti method usually yields good results. The Ponseti method can also be used as a first line treatment in older and more resistant cases. In such severe and neglected cases bony surgery in the form of foot osteotomies and arthrodesis is usually indicated. It is usually accompanied by soft tissue surgery in the form of release of contracted tendon and capsular structures. In older patients near skeletal maturity joint fusion or arthrodesis may be indicated as well. Less frequent patients with arthrogryposis may develop congenital vertical talus also known as rocker bottom foot. Similarly, congenital vertical talus is classically managed by serial casting according to the reversed Ponseti method. Resistant or recurrent cases may be offered an extensive soft tissue release. However this is fraught with risk of foot stiffness and pain in the long term. Talectomy or excision of the talus to give room for creation of plantigrade foot has been practiced. Naviculectomy or midtarsal resection arthroplasty represents a less invasive option with satisfactory short-term results.
